Medical Update Form

Campers will have provided basic medical information near the time of booking.  However, we want to be absolutely sure that the information we have is up to date when camp starts.   

The Directors of the holiday weeks will be in touch and ask you either to complete an online document or they will send a copy of the form they would like you to complete and bring to camp. 

Please make sure you complete their request as soon as possible so that we can be well prepared to look after each camper well.
